Dear Parent/ Guardian,

Thank you for entrusting your child/children to our care.  Please review the Health Office Notices and the forms relevant to your child's grade.  
 
Absence:
If your child is going to be absent from school due to illness or medical/dental procedures, please notify the Health Office at 516-922-4233 Ext. 3166 or the Main Office at 516-922-4233 ext. 3101. You can also email your child's teacher. If we do not hear from you regarding your child's absence you will receive a robocall instructing you to contact us. 
 
Health Examination Requirements:
-The NYS DOH requires health examinations for all new entrants and students entering PK, K, 1st, 3rd, 5th, and 7th grade.  
- **We can only accept the NYS Required Health Exam Form (attaching the doctor's own form to the NYS form will not be accepted).  
- A copy of your child's health examination form must be provided to the school within 30 days from when your child first starts school as a new student, and when entering Kindergarten, 1st, 3rd, 5th, and 7th grade.
**Please make sure forms are complete, signed, and stamped by your child's health care provider along with a copy of their most up-to-date immunization record. To email health records please communicate with the health office prior to sending forms by email with "Health Forms" in the subject line.
 
- Immunization updates are required for all new entrants and students entering grades; PK, K, 6th, and 7th  within 14 days of the start of school (see grade level Health Office Notices for information on grade level immunization requirements)
 
- If your child has an appointment for an exam during the school year that is after the first 30 days of school, please notify the Health Office of the date of the scheduled examination.
 
-Throughout the year, please keep the health office informed of any diagnosed infectious diseases such as; the Flu, Strep Throat, Mononucleosis, Conjunctivitis (Pink Eye), Tinea (wring worm), Fifth’s disease, head lice, COVID-19 or exposure to COVID,  as well as any changes in your child’s health status or medications.
 
- Medications
1) Medication and related paperwork should be turned in together in person by a parent/guardian. Please do not send medications to school with students without their doctor's written approval to do so and without speaking directly with the health office staff first.
2) medication drop-off appointments will need to be scheduled to receive and go over medications and related paperwork. Please contact the health office ahead of time to set up an appointment for medication drop-off. - Sports
Health Exams should be done and dated within 12 months of the start of each sports season. Also, if the health exam is more than 30 old at the start of the sports season the Interval Health History Form for Athletics needs to be completed by a parent. Then the athlete will need to be cleared by the health office at the start of each sports season.
